Your account must have been created by your system administrator!

• Login to your account using your username and password
• If you forget your password, click on 'Forgot password' button located at the bottom of the login panel
• Provide the email address you used during account creation
• The system will send email message to you with a reset password link, follow the link and reset the password.
• On successful login, you will be redirected to your dashbord
• Control menus will be displayed by the left hand side of the dashboard page
• These menus are:
  • Dashboard
  • Questionnaire (with sub-menus: Create Question, List Question, Create Questionnaire, Share Questionnaire and List Questionnaire)
  • Reports (with sub-menu: Reports)
  • Logout
• To create a question, go to Create Question, fill the form appropritely and save
• To view all questions created, go to List Question
• To create a Questionnaire, go to Create Questionnaire, fill the form appropritely and save
• To view all Questionnaire created, go to List Questionnaire
• To share questionnaire, list questionnaire, view the questionnare that you intend to share and then click share. This takes you to where you can share the questionnaire link to the users via email and/or Text Message(SMS)
• In order to generate report for what the users have submitted, go to Reports. This prompts a form, use it to filter your search and submit. This search is based on the concept of EDT model with two different service categories i.e. Expected Services and Perceived Services
• Use the Logout menu to exit from the system.
